• (3) Copies printed and filled Request for Live Scan paper form*, e.g. BCIA 8106
• A Valid Driver's License or Passport
• Prepaid Live Scan Service Coupon from the Certifix Website
• Order the service from this page!
Our Mobile Live Scan fee is $49 and in addition to Live Scan Service fees, e.g. State Fees + Rolling Fees.
* If you do not have access to a printer but have the form in an electronic format, we will be happy to print this out for you and bring it to your appointment.
